Form 49A : PAN card application form
Form 49 A is required to be submitted to the income tax department to get a PAN number for individual. So form 49A is basically a PAN request application form for allotment of Permanent Account Number under section 139A of the Income-tax Act, 1961.
It is a simple form and only proof of identity and proof of address is required to be submitted along with the application form at any income tax office in India. The PAN card is sent on the communication address and there is no need to go and collect the same from the Income tax office.
PAN number is becoming mandatory day by day in most transactions and as per new income tax rules in 2010 income tax authorities can impose up to 20% of tax deducted at source (TDS) if the recipient of the income fails to provide his PAN to the agency giving this amount.

* Documents submitted, as proof of address for serial numbers 1 to 7 should not be more than six months old from the date of application.
Important Note
- Those individuals / companies already allotted a ten-digit alphanumeric PAN shall not apply again, as having or using more than one PAN is illegal as per Income tax laws in India
- Other than this request for a new PAN card with the same PAN or/and changes or correction in PAN data can be made by filling up the form for ‘Request for New PAN Card or/and Changes or Correction in PAN Data’.
